Who we are
History
Founded in 1999 as a 501C3 non-profit, the program was established to meet the needs of elders that live in their own homes. By enhancing and protecting the health and well being of elders, families and caregivers in our community, our program provides a means for people to improve and maintain wellness, stay in their homes they love, avert or delay nursing home placement and effectively prevent health care problems before they escalate.

Mission

The mission of LAHBNP is to assist older persons, helping them to live safely and independently in their own homes and communities. Recognizing the capability of local neighborhoods and communities to integrate the social and in-home services needed by the elderly and disabled, our goal is to help seniors and disabled people avoid unnecessary emergency room visits and premature entry into the long term care setting.

Vision
​
In meeting its mission, the LAH/BNP draws upon a network of wonderful, dedicated volunteers under the direction of three part-time staff. LAH/BNP’s Board of Directors is made up of 9 members who bring expertise in health care, finance, administration, education and ministry to their oversight of the program. We serve people 60 years of age or older and with disabilities in Granite Falls and the surrounding rural community in SW Minnesota. GF-LAH/BNP coordinates with several area organizations and agencies in order to reach the most isolated and vulnerable.
